Knee Brace with Patella Gel Pad: How an Open-Patella Brace Should Fit

Quick answer: A knee brace with a patella gel pad has an opening at the front for your kneecap, framed by a soft gel ring. The ring's job is positioning: it gives the brace a fixed point around the kneecap so it stays centered while you bend, walk and move. For the right fit, the kneecap should sit inside the ring with the ring's edge resting evenly around it, never on top of it. Start by measuring around your thigh 4 inches above the kneecap.

What is a patella gel pad?

"Patella" is the anatomical name for the kneecap. On a knee brace, a patella gel pad is a ring of gel built into the front of the brace around an opening for the kneecap. That's why you'll also see this design called an open-patella brace or a knee brace with a hole for the kneecap.

The gel ring does two things:

  • It marks the center. When the kneecap sits inside the ring, the brace is lined up correctly, and the side stabilizers land on the inner and outer sides of the knee where they belong.
  • It helps the brace stay centered as the knee bends and straightens, so you aren't re-adjusting it all day.

Open-patella vs. closed-front braces

Open-patella with gel ring Closed front (no opening)
Front of the knee Opening framed by a gel ring Solid fabric
Lining up the brace Easy: put the kneecap inside the ring You judge the center by feel
Feel when bending Fabric doesn't press flat across the kneecap Fabric stretches across the kneecap
Often paired with Side stabilizers and adjustable straps Pull-on sleeves

Neither style is right for everyone. People who want a brace that lines up the same way every time often prefer the open-patella design. If you mainly want light, even compression, a pull-on sleeve may be all you need. See knee sleeve vs. knee brace for a side-by-side comparison.

Where the gel ring should sit

When the brace fits well:

  1. The kneecap is inside the opening, with the ring resting evenly around its edge.
  2. The ring is not riding up over the top of the kneecap or sliding down below it.
  3. The side stabilizers run straight up the inner and outer sides of the knee.
  4. The straps lie flat, with no twisting or bunching.

Bend your knee a few times after you put the brace on. The ring should move with the kneecap and come back to the same place when you straighten your leg.

How to put on an open-patella brace

  1. Start with bare skin. The silicone grip strips on the inside hold best on bare skin.
  2. Line up the opening first. Before fastening anything, center the gel ring around your kneecap with your leg slightly bent.
  3. Fasten the middle straps, then the rest. Close the straps nearest the knee first so the ring stays in place, then the upper and lower straps.
  4. Check the fit. One finger should slide under each strap. The brace should feel snug, not tight.
  5. Do a test walk and a few slow bends. Re-center the ring if it has moved.

If the gel ring won't stay centered

What you notice Likely cause What to try
Ring drifts up over the kneecap Upper straps tighter than lower straps Loosen the upper straps and re-fasten them evenly
Ring slides down when you walk Brace too loose, or worn over fabric Wear on bare skin and snug the middle straps
Ring sits off to one side Brace rotated when it went on Take it off, center the ring first, then fasten
Ring feels like it presses on the kneecap Wrong size or ring not centered Re-center; check your size on the size chart

Size decides where the ring lands

The gel ring only sits right if the brace is the right size. A brace that is too big can slide and pull the ring off center. One that is too small can stretch the opening out of shape.

  • Measure around your thigh 4 inches (10 cm) above the top of the kneecap.
  • OS fits up to 21.5 inches (54.6 cm). OS+ fits up to 25 inches (63.5 cm).
  • Between sizes? Go OS+.

For bigger thighs, see the knee brace for larger legs guide. The full chart is on the knee brace size chart page.

Gel pad plus side stabilizers

A gel ring keeps the brace centered at the front. Side stabilizers add structure along the inner and outer sides. Together, they give you a brace that lines up the same way each time and feels firmer on the sides than a sleeve. Read more about what side stabilizers on a knee brace do.

Frequently asked questions

What is a patella gel pad on a knee brace?

It's a ring of gel around an opening at the front of the brace, where your kneecap sits. It helps you line the brace up correctly and keeps it centered as your knee bends.

What does open patella mean on a knee brace?

It means the brace has an opening at the front for the kneecap instead of solid fabric. On the Dr. Brace Elite, that opening is framed by a gel ring.

Where should the gel ring sit?

Your kneecap should sit inside the opening, with the ring resting evenly around its edge. It shouldn't ride up over the kneecap or slide below it.

Why does the gel pad on my knee brace move up or down?

Usually the straps are fastened unevenly, the brace is the wrong size, or it's worn over clothing. Wear it on bare skin, center the ring first, then fasten the middle straps before the others.

Is an open-patella brace better than a sleeve?

It depends on what you want. An open-patella brace with side stabilizers and straps gives more structure and lines up the same way each time. A pull-on sleeve gives lighter, even compression.

What size Dr. Brace do I need?

Measure around your thigh 4 inches (10 cm) above the kneecap. OS fits up to 21.5 inches (54.6 cm) and OS+ up to 25 inches (63.5 cm). If you are between sizes, go OS+.
